Dr. Afzali has led initiatives in sepsis care, antibiotic stewardship, diagnostic workflow redesign, and point-of-care ultrasound implementation, with a consistent focus on evidence-based practice, appropriate escalation of care, and operational efficiency. He has served on national professional committees and has trained physicians both domestically and internationally in countries like Guatemala and Afghanistan.

He is a Fellow of the American College of Emergency Physicians, board-certified by the American Board of Emergency Medicine. He is fluent in English, Farsi, Pashto and Spanish.

APPROACH
Dr. Afzali's approach is characterized by:

  • Deliberate attention to early clinical signals when medically relevant
  • Careful synthesis of clinical findings rather than reliance on isolated tests
  • Long-term physician–patient relationships that support continuity and accountability
  • Active coordination across specialists, health systems, and care settings

This model is designed for patients who value a physician who can interpret complexity, anticipate needs, and guide care thoughtfully over time within an increasingly fragmented healthcare environment.

Dr. Leeda Rashid, MD, MPH, ABFM

Dr. Leeda Rashid is a board-certified physician and public health professional with extensive experience across primary care, hospital medicine, and complex healthcare systems. She has been in practice for over 13 years and brings a rare combination of frontline clinical practice, experience in digital health and artificial intelligence, federal health policy advisory work, healthcare research, and technology evaluation.

Dr. Rashid has served as a Senior Physician Advisor at the FDA within the U.S. Department of Health and Human Services, providing clinical and regulatory expertise related to technology-enabled healthcare delivery, and the use of safe and effective devices in healthcare. Working across agencies including CMS, ONC, AHRQ, HRSA, NIH, and SAMHSA, she contributed to policy development and regulatory science initiatives that shape how medical devices are delivered, evaluated, and used in patient care. This experience informs her ability to help patients navigate care decisions, coverage considerations, and evolving models of care in a complex and fragmented healthcare system.

In addition to her policy work, Dr. Rashid has held senior leadership roles in healthcare technology companies, supporting the development and evaluation of data-driven clinical tools and care coordination platforms. Her work has consistently emphasized alignment between clinical evidence, regulatory requirements, and real-world care delivery; principles that guide her approach to individualized patient care.

Dr. Rashid has contributed to peer-reviewed research spanning digital health, population health, regulatory science, and global health systems. Her research has examined the use of digital health technologies, including artificial intelligence, in the management of chronic diseases and patient preferences. She serves as a peer reviewer for the British Medical Journal Digital Health & AI, evaluating research focused on the appropriate and evidence-based use of artificial intelligence in healthcare.

Clinically, Dr. Rashid has practiced across primary care, hospital medicine and skilled nursing settings, caring for patients with varying degrees of medical complexity across diverse populations. This breadth of experience supports thoughtful longitudinal care, appropriate escalation, and coordination across specialties and care environments.

Dr. Rashid holds an MD from the Oregon Health and Science University, and an MPH from Harvard T.H. Chan School of Public Health, and is board-certified by the American Board of Family Medicine.  She is fluent in English and Farsi.

APPROACH
Dr. Rashid's approach emphasizes:

  • Personalized care informed by detailed assessment and longitudinal health data
  • Selective use of digital tools and advanced diagnostics when clinically appropriate
  • Focus on functional health, longevity, and long-term risk reduction
  • Integration of high-quality scientific evidence into day-to-day clinical decisions
  • Preference for clarity and relevance over excessive or non-informative testing

Her practice is intended for patients seeking a sustained physician partnership; one defined by careful analysis, measured use of technology, and a long-term view of health and longevity.
Heres what our patients are saying about us:

"Dr. Afzali is very skilled and very patient focused. He is direct in his communication and treats his patients as integrated wholes rather than the sum of their parts. In an exploding ER, he is the epitome of grace under pressure. A rare and gifted doctor"

"Quick but not rushed, focused but thorough, forward but somehow compassionate and caring. Then two days later, i get a follow up call. =)"

"
I have been using this service for almost a year now and have been pleasantly surprised by how pleasantly non-complex proper, attentive, contextualized medical care can actually be! Even being based in Southern California, they have been able to accommodate my needs but importantly, they have been proactive to ensure that my 40s are a period to thrive in, not respond or recover in. Thanks PYCC Team!

"I am writing to formally recognize and commend Dr Edris Afzali and Nurse Harumi K.  for the outstanding care they provided me. Dr Afzali stepped into a highly tense situaton and immediately brought a sense of calm and control to the room. He gracefully de-escalated the environment, took over my care and communicated with me clearly and throroughly. He actively listended to me and treated me with the utmost dignity and respect. After a very traumatic start to my visit, Dr Afzali's professional, empathetic and patient-centered bedside manner made me feel safe again."
